Art Show Consent Form

Thank you for a marvelous Arty Party!  We would like to have your child's artwork on the wall for our August show running August 3 - 27.  It is titled, "Hancock County Scenes" and Arty Party has definitely been part of a Hancock County scene.  Please fill out the brief form below indicating that we have your permission to show their artwork, name and photograph.  If you prefer we display your child's artwork without their name or photograph, we will be glad to do that. Just check below. Our Artists' Reception is August 12 from 6:30 - 8:30 PM.
